Subject: Urgent Psychiatric Referral – Ms. Claire Cobert (DOB: 27/04/1995)



Dear Dr. Vallois,


I am writing to refer Ms. Claire Cobert for urgent psychiatric evaluation and follow-up care.


Ms. Cobert presented to my clinic today in a state of acute emotional distress. She is undergoing what appears to be a severe psychological crisis following the recent end of an intimate relationship with her former girlfriend. During the consultation, Ms. Cobert disclosed (without entering into specifics, as she is concerned that this may be a potential life-threatening situation) that she may have been the victim of repeated sexual assaults. Her account remains fragmented and vague at this stage, and she was visibly reluctant to elaborate on the events.


She is currently exhibiting symptoms indicative of a major depressive episode: persistent insomnia, emotional withdrawal, episodes of uncontrollable crying, diminished appetite, and marked feelings of hopelessness. There is also concern regarding dissociative episodes and suicidal ideation, though she denied any current plans to harm herself.


Given the gravity of her condition, I have initiated immediate medical treatment, including a low-dose SSRI (Sertraline 50mg) and advised strict psychological monitoring. She has agreed, with some hesitation, to this referral for psychiatric assessment and ongoing therapy.


Her case requires urgent review, both for diagnosis and to establish a safe and structured treatment plan moving forward.


Thank you for accepting this referral. This seems to be quite an urgent matter, as I strongly believe this young girl is in danger.
