On the ninth of January, at eleven twenty-five in the morning,
was born at the Montpellier Lapeyronie University Hospital – Maternity Ward,
Charlotte Taylor Alison Margaret Kominsky,
female, of French and British nationality,
Daughter of:
Mr Adam Kominsky, born on the 2nd of November 1971 in Saint-Clément-de-Rivière (Hérault), profession: banking clerk,
and
Ms Amelia Whiteridge, born on the 9th of May 1976 in Manchester (United Kingdom), profession: student,
Both residing together at 23, rue du Faubourg Saint-Jaumes, 34000 Montpellier.
The declaration of birth was made this same day at two o’clock in the afternoon by the father, Adam Kominsky, aged twenty-three years, in the presence of Mireille Tournier, certified midwife, and Louis Delgado, officer of the civil registry.
After reading, the present certificate was signed by all parties.
Drawn up and signed in the civil registry office of Montpellier, on the ninth of January 1995.
